Information About 401K Retirement Plan

Your Employer is sponsoring this Plan so that you may save for retirement. As a Participant under the Plan, you may elect to reduce your compensation by a specific percentage or dollar amount and have that amount contributed to the Plan on a pre-tax basis.


Your employer will match 100% for the first 3% of your contribution and 50% after that up to 5% contribution. For example:


  • 1% contribution from your payroll will be matched with 1% by employer
  • 2% contribution from your payroll will be matched with 2% by employer
  • 3% contribution from your payroll will be matched with 3% by employer
  • 4% contribution from your payroll will be matched with 3.5% by employer
  • 5% and up contribution from your payroll will be matched with 4% by employer

You are always 100% vested - which means that you are entitled to all of the amounts.


You may become a "Participant" in the Plan once you have satisfied the eligibility requirements:
- one year of employment with at least 1000 hours of service
- reached your "Entry Date" (January 1st or July 1st)


You will be able to direct the investment of your entire interest in the Plan.

Summary Plan Description ("SPD") contains information regarding when you may become eligible to participate in the Plan, your Plan benefits, your distribution options, and many other features of the Plan. You should take the time to read SPD to get a better understanding of your rights and obligations under the Plan.

How to enroll in 401k plan?

Eligible employee of Patriot Home Care Inc. can enroll to 401k plan in two ways:


  1. Online (see instructions below)
  2. Phone: 1-800-923-4567 (Customer Service Representative will help you to enroll 24 hours 7 days a week)

ONLINE ENROLLMENT STEPS:


  • Go to WWW.TDARETIREMENTPLAN.COM/MYFUTURE
  • Click “Log In” in the middle of the screen
  • Under “User ID” enter your social security number (no dashes)
  • Under “Password” enter the last four digits of your Social Security Number, Click “Login”.
  • If you asked verification PIN, please provide your cell phone number and/or email to your employer and PIN will be texted to your cell phone or email.
  • Once you have gained access to the system, you will be given the opportunity to change your log in credentials. Since this is your first time logging in, you will be asked to provide four security questions and answers. These will be used when you are retrieving a password or logging in from an unfamiliar or public computer. Choose which questions you would like to be asked in the future and provide the answers in the adjacent answer boxes. The answers you provide are case sensitive. When finished, click on the “Submit” button. Click “Next” after each step below is completed:
  • Account Setup: This page will include your personal data on file, including but not limited to the following: name, date of birth, date of hire, email address and home address. You may make changes to your contact information on this section of the website.
  • Beneficiaries: This step allows you to enter your beneficiary designations. Click “Add Additional Beneficiary” if you have more than one beneficiary.
  • Contribution Election: This step allows you to enter the amount you wish to contribute per pay period either as a percentage or a flat dollar amount. There is also a paycheck calculator included so that you can analyze the impact of any contribution level on your take home pay. If you have made contributions to another employer’s retirement plan, please enter that information on this screen.

    - 3% contribution from your payroll will be matched with 3% by employer
    - 4% contribution from your payroll will be matched with 3.5% by employer
    - 5% and up contribution from your payroll will be matched with 4% by employer (maximum match)
  • Investment Selection: This step allows you to review fund performance and prospectus information and select your investments. Please read the prospectuses of the funds you’ve chosen before investing. Your total allocation percentages must equal 100%. If you choose to not select investments at this time, any contributions made will be invested in the Default Investment Alternatives (Target Retirement Funds). Those funds designed for investors planning to retire and leave the workforce in or within a few years of the target year. For example: for participant who was born on 1980 with retirement at 65 years old, the Vanguard Target Fund 2045 (year of retirement) will be automatically chosen.
  • Confirmation: This final screen allows you to review the information entered into the system and make any necessary changes to the content. Once you are comfortable with the information, click “Finish”.
